MedusaGPT
Why?

Furosemide is a high-ceiling diuretic because the TAL handles about 25% of filtered Na⁺, and the segments after it have too little spare capacity to catch it. A thiazide blocks the DCT, which handles only 5 to 10%, so its ceiling is far lower.

Each effect below traces back to that one blocked carrier:

Effect
Why it happens
Large diuresis
A quarter of the Na⁺ load escapes to segments that can't reabsorb most of it
Can't concentrate urine
Less NaCl reaches the medulla, so the gradient washes out
Ca²⁺ and Mg²⁺ loss
K⁺ stops recycling, so the lumen-positive voltage that drives them is lost
Hypokalemia
More Na⁺ and flow reach the collecting duct while aldosterone is high
Metabolic alkalosis
Aldosterone drives H⁺ secretion, and volume contraction concentrates HCO₃⁻
Renin ↑
The macula densa senses NaCl through NKCC2 too, so it reads low NaCl

Card Match

The slides go up after lecture.

Your lecturer posts 38 pages on antiarrhythmics. Somewhere in the AnKing deck are the cards that match, most of them still suspended.

Give Medusa the PDF. It searches your cards on this computer, grades the best 400, and sorts the matches by the lecture's own topics, with page numbers.

  1. Drag the PDF onto Anki, or use Medusa → Find cards for a document…
  2. Press Find cards (up to 17 credits).
  3. Press Unsuspend. The yellow rows are suspended cards.

PDF, PowerPoint, Word, text or Markdown. The file itself isn't uploaded. About 10 to 25 credits a document. Works on any deck, not only AnKing.

With Medusa
Medusa Lessons: Staphylococcus aureus
Step 1 of 6

The big picture

S. aureus is a gram-positive coccus that grows in clusters. It is catalase-positive and coagulase-positive, which separates it from the other staph.

Your cards also say S. epidermidis is coagulase-positive. That one is checked at the end.

Step 2 of 6

How it causes disease

Protein A binds the Fc region of IgG, so antibodies can't opsonize the bacterium.

It is the most common cause of acute infective endocarditis, including tricuspid valve disease in people who inject drugs.

Beyond your cards

Panton-Valentine leukocidin is a pore-forming toxin linked to necrotizing pneumonia and skin abscesses in community-acquired MRSA. None of your cards cover it.

Step 3 of 6

Toxin syndromes

TSST-1 is a superantigen. It cross-links MHC II and the T-cell receptor and causes toxic shock syndrome.

Exfoliative toxin cleaves desmoglein 1 and causes scalded skin syndrome.

Food poisoning starts within hours because the enterotoxin is already made when the food is eaten.

Step 4 of 6

Resistance and treatment

MRSA carries mecA, which encodes PBP2a, a penicillin-binding protein that beta-lactams can't bind well.

Vancomycin is first-line for serious MRSA infections.

Step 5 of 6

Check yourself

A 24-year-old woman has fever, hypotension and a diffuse red macular rash three days after starting her period. Two weeks later the skin on her palms peels. How does the toxin responsible cause disease?

This is toxic shock syndrome from TSST-1, a superantigen. Cross-linking MHC II with the T-cell receptor activates many T cells at once and releases IL-1, IL-2, IFN-γ and TNF-α.
Step 6 of 6

Check your cards

Staphylococcus epidermidis is coagulase-positive.

May be wrong: S. epidermidis is coagulase-negative. S. aureus is the coagulase-positive staph.

Why Medusa

A tutor that reads before it answers.

The library

reference passages. MedusaGPT reads the relevant ones before it answers, so you get the mechanism and the specifics.

Asked from the back of a card
“Why do loop diuretics waste calcium when thiazides save it?”

Furosemide blocks NKCC2 in the thick ascending limb. K⁺ stops recycling into the lumen, the lumen-positive voltage goes away, and the calcium it was pushing between the cells leaves in the urine. A thiazide blocks NCC in the distal tubule instead. With less Na⁺ inside the cell, the basolateral Na⁺/Ca²⁺ exchanger runs faster, so the cell pulls more Ca²⁺ in from the lumen.
